How do I make a form that adds rows to a database?

Add a Form view, tick the columns to ask in the view's options, and each Submit adds a row with those answers.

Updated October 26, 2026

Add a Form view to a database and tick the columns it asks. Your team fills it in, clicks Submit, and each answer set becomes a new row.

Also called: form, survey, intake form, questionnaire, formulaire, sondage.

Step 1: Add a Form view

Click + beside the view tabs and pick Form. It starts with one question, the row's name, marked Required.

Click: + › Form

Step 2: Pick the questions

Open the sliders button. Under Questions, tick each column to ask, and tick Required for the ones that must be answered. A select shows as a list to choose from, a date as a day field, and a checkbox as a box to tick.

Click: sliders button › Questions

Step 3: Fill it in

Answer the questions and click Submit. You'll see "Saved. Thanks!" and the new row is in the database. Click Answer again for another one.

Click: Submit

If it doesn't work

  • Submit stays grey: a required question has no answer yet.

  • "Answer every required question.": fill in each question marked with a red *.

  • "This form has no questions yet. Add some in the view's options.": tick at least one column under Questions.

  • Only teammates who can edit the database can submit the form.
